- Gift cards may be combined with other offers or discounts, including Alle Points.  However, only one gift card offer from TitleMD can be used per person.

- Present the Gift card to apply the listed amount exclusively toward your next Filler treatment. 

- Gift cards do not expire; and cannot be applied to past transactions or redeemed or exchanged for cash.

- Must use the complete value of each gift card in a single transaction.

- Use of this Gift card is subject to an independent face to face examination by the treating physician.   Not all people are candidates to receive certain treatments.  If the treating physician determines that the recipient of this gift card is not a medically appropriate candidate for treatment, or if treatment is contraindicated based on the recipient’s medical history or presenting conditions, the original purchase price of the card will be refunded.  The gift card is otherwise not redeemable for cash nor refundable.
