- Treatment(s) must be completed by September 30, 2019.

- The above specials may be purchased for either the personal use of the individual purchaser or may be purchased as a gift.  Certificate is transferable.

-  A Botox site consists of either: horizontal forehead lines, vertical lines between eyebrows, or

   crows feet lines at corners of the eyes.

-  All Botox treatments must be completed in a single session.  

-  Multiple Juvederm syringes can be used on different treatment sessions.

-  You will receive an e-mail confirmation after your purchase.

-  Must be at least 18 years old to purchase.

-  Not all people are candidates to receive certain treatments.  Should the treating physician make the determination that the purchaser of this discount is not a medically appropriate candidate for such treatment or that the treatment is contraindicated based upon the bearer’s presenting conditions and/or history, then the original purchase price of this certificate will be refunded.  The certificate is otherwise not redeemable for cash nor refundable.
